I, the undersigned, for myself and my heirs and legal representatives forever release the Talita Kum, L.L.C. DBA 'Zante', its employees, agents, their respective heirs and legal representatives from any damages, claims, demands, rights and causes of action of whatever kind or nature based upon personal injuries, death or property damage to me, whether or not caused by negligence, errors or omissions of Talita Kum, L.L.C. DBA 'Zante' its employees or agents or third parties or myself.

I, for myself and for my heirs and legal representatives agree to indemnify Talita Kum, L.L.C. DBA 'Zante', its employees, agents their respective heirs and legal representatives against, and save them harmless from all damages, actions, causes of action, claims, judgements, executions, debts, cots of litigation and attorney fees which may in any way arise out of or result from my participating in Talita Kum, L.L.C. DBA 'Zante' activities even if the damages are caused by the negligence of Talita Kum, L.L.C. DBA 'Zante', its employees or agents or third parties or myself.

I certify that I have read the foregoing instrument, that I understand its terms and conditions, that I make this waiver voluntarily, and that I have not relied upon any representatives made by Talita Kum, L.L.C. DBA 'Zante', or its officers, agents, officials, or employees in signing this document. 

I further certify that I am an adult.

If any one or more of the provisions of this waiver shall be for any reason whatsoever held invalid, then such provisions shall be deemed severable from the remaining provisions and shall in no way affect the validity or enforceability of the other provisions.

The laws of the State of Texas govern this waiver, without regard to choice of law principles.
